RESERVATION POLICIES:

  • To reserve Oak Park Varsity Club (OPVC), members must use the online scheduling app provided by OPVC management.

  • Non-members can reserve the facility but must first request access via email using the following email address info@varsitycluboakpark.com

  • Members can reserve up to 6 hours of private/semi-private field time.

  • Members and non members are NOT permitted to book more than one session per day.

CANCELLATION POLICY:

  • Members and Guests are required to cancel any reservation that they are not able to use.

  • Members and Guests are asked to cancel reservations through the scheduling app.

  • Members and Guests that do not cancel reservations at least 24 hours in advance of their scheduled session will be considered a no show and will be a assessed a $75 fine for each occurrence.

  • Members and Guests are permitted to cancel reservations twice each month without penalty. For each cancellation beyond the second time each month, members or guests will be fines $25 for each incident.

  • Members and Guests that overbook the number permitted sessions per month, will be fined $75 per incident.

  • Members and Guests that overstay their reservation by more than 5 minutes will be assessed a $25 fee for each incident.

  • Members and Guests that bring more than the maximum number of allowed attendees will be assessed a $25 fee for each additional attendee.

    • In all incidents listed above, members could also have membership suspended for 30 days.

FACILITIES:

All members, guests, trainers and participants are required to maintain the high quality and standard of the facility by using the equipment, playing surface, walls or any other items on premise in the manner that is intended and described. The OPVC HOUSE RULES include, but not limited to:

  • Turning off the lights when leaving the building,

  • No food is to be eaten in side of the OPVC unless approved ahead of time by OPVC management.

  • Food served during parties is only permitted in the party room located in the southwest area.

  • All bottles or trash should be disposed of in the trash containers located in OPVC, or carried out when departing.

  • Equipment, including soccer goals, pitching mound, batting nets, Shaped Screen, etc should not be moved from the designated area.

  • All people using OPVC should avoid throwing or kicking balls of any kind against non padded walls.

  • All people using OPVC should avoid hanging from the soccer goal cross bar.

  • The replacement of damaged equipment or facility structures within OPVC due to clear misuse will be the responsibility of the parties involved.

  • All people using OPVC are asked to wrap their session 5 minutes prior to scheduled end time to ensure that the next group can being their session on time.

    Covid Facility Rules

     

    ·      Do not enter the facility if you have had a fever, cough, shortness of breath or have had exposure to someone with symptoms of Covid-19

    ·      All coaches, trainers and spectators must wear a mask at all times

    ·      Participants please were a mask when not engaged in vigorous activity when possible 

    ·      Do your best to maintain at least 6 feet of distance between yourself and others on the premises

    ·      Regularly wash or sanitize your hands. It is mandatory to do so after using the bathroom, coughing, sneezing or after exposure to any secretion. 

    ·      Be safe, use common sense and be respectful to the safety needs and requests of all.

GUESTS

  • Any person that reserves OPVC is solely responsible for the well being any family members, guests, youth athletes or clients invite to participate in activities at the Oak Park Varsity Club.

  • All guests are required to follow the same rules and guidelines as OPVC members.

    •          All children under the age of 14 must be accompanied by an adult.

    •          Any teen 14-17 must have signed consent from a legal guardian. This can be achieved by acknowledging the terms and conditions for any program, membership or field reservation completed online.

    •          Any unaccompanied teen, aged 14-15, is allowed to occupy Varsity Club for approved activities, no later than 8 pm.

    •          Any unaccompanied teen, aged 16-17, is allowed to occupy Varsity Club for approved activities not later than 10pm.

    •          Any individual using the space for activities or behaviors that are not of permitted will face fines* and possible membership revocation, if applicable.

    •          Here is a list of activities and behaviors that are not permitted within Varsity Club

    o          Smoking, using street drugs, improperly using prescribed medicine, sexual activity, nudity, any form of harassment, intentionally damaging property, returning to the facility after hours, using the facility for personal entertainment purposes, conducting any illegal activity, fighting, streaming pornography, illegal downloads while logged on to Varsity Club WiFi, entering unauthorized areas, sharing entry codes with individuals outside of your immediate family or staff.

    o          Reconfiguring the permanent features in space including the protective nets, batting cage, wall pads and cameras

    o          This is by no means an exhaustive list but it gives an indication of the activities and behaviors that are not permitted within Varsity Club.

    * Fines are up to $1,000 plus cost of damages when applicable.

     

ACKNOWLEDGEMENT OF WAIVER

In consideration of being allowed to participate in any way in Oak Park Varsity Club athletic sports program, related events and activities, the undersigned acknowledges, appreciates, and agrees that:

  1. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,

  2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,

  3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,

  4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Oak Park Varsity Club their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

  5. I agree to maintain the high quality and standard of the facility by using the equipment, playing surface, walls or any other items on premise in the manner that is intended and described.

  6. I agree to be solely responsible for the well being any family members, guests, youth athletes or clients that I invite to participate in activities at the Oak Park Varsity Club.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
